By checking this box I hearby certify that I am over the age of 18 and that I and any attendees I am registering through this form have read, understand, and agree to the terms of the NorthWoods Activity Liability Waiver. If an attendee I am registering is under the age of 18, I affirm that I am a legal parent or guardian and hearby affirm that I give permission for this child to attend and participate in this event under the terms laid out in the Liability Waiver and that I give them permission to use a shuttle without me, and to wait at the NorthWoods campus and lodge. I understand that said child will not be supervised while they wait for my arrival. I also acknowledge the Refund Policy for this fundraising event: In the case of event cancellation by NorthWoods, registrants will be refunded their entry fee, minus $25 for costs incurred. In the case of individual cancellation of registration by an attendee, a refund will not be granted.
